Privacy Policy

1. Overview and scope

This policy describes information collected through the website of Tate Medical Associates of North Georgia, part of Privia Medical Group. It explains what we collect, how we use and share it, and your choices. It is separate from the Notice of Privacy Practices for your medical records.

2. Information we collect

When you use a website form, we collect the information you provide, such as your name, phone number, email address, preferred appointment date or time, provider or service selections, preferred therapy, and text-message consent. Some forms have message, goals or current-medications boxes. We collect any free-text message or health information you choose to enter.

Our website and service providers may also collect your IP address, device and browser information, pages visited, referring website, and advertising information such as the Google click ID (gclid) and UTM campaign parameters. Cookies and browser storage help operate the site and measure visits and advertising. Advertising attribution may be kept in browser storage for up to 30 days.

3. Website forms are not for detailed medical information

Our website forms are not a secure channel for medical records and are not intended for detailed health information. Please do not submit sensitive health details, such as diagnoses, medications, test results or insurance IDs, through them, even if a form includes a health-related text box. Share those details with us by phone, in person or through the patient portal. The website uses HTTPS, but this does not make these forms a secure medical-records channel.

4. How we use information

We use website information to respond to your request, contact you, schedule appointments, send requested confirmations and follow-ups, operate and secure the website, and measure the effectiveness of our website and advertising. We send text messages about your request only if you consented.

5. How we share information

Our staff use form submissions to respond to you. Information may be shared through internal team messaging and email notifications, a lead-tracking database and CRM, an email delivery service, and website hosting and database providers. Staff email notifications may contain your contact details, selections and the free text you submitted. Team messaging and CRM records include contact and request-tracking information; they do not include your free-text goals, medications or message.

When you consent to text messages, we use Twilio to deliver them to the phone number you provided. Google receives information through advertising tags and, when configured, Google Analytics. Browser conversion tags send the conversion destination and a request identifier, not your submitted email, phone number or health text. For eligible advertising conversions, our server sends Google the Google click ID (gclid), conversion timestamp, event source and advertising account and conversion-action identifiers. This upload does not include email, phone, hashed contact details or health text.

We may also disclose information as required by law or to protect safety. We do not sell your personal information, and we do not share it with third parties for their own marketing. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.

6. Cookies and tracking technologies

We use Google advertising tags and Google Analytics to measure visits and actions such as appointment-link clicks and form submissions. These services may use cookies and collect device, browser, page and advertising information. Embedded content, including Google Maps and a Google Drive video, and externally loaded resources such as fonts may also receive your IP address and browser information and use their own cookies.

You can manage or block cookies in your browser and clear browser storage. Some website functions may not work as expected if you block these technologies. You can manage advertising preferences through Google Ads Settings and use the Google Analytics opt-out add-on to limit Google Analytics collection where it is used. We do not currently respond to Do Not Track signals.

7. Text messages

If you choose the text-message consent option on a Metabolic Reset form, we may text you about your request at the number you provided. Text messaging is not required to submit a request. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. You can also call our office for assistance. Other website lead forms do not enroll you in this text-back service.

8. Security

We use reasonable administrative and technical safeguards to protect website information. No method of internet transmission or electronic storage is 100% secure. Please use the patient portal, phone or an in-person visit for detailed medical information.

9. Retention

We keep form submissions only as long as needed to respond to your request, schedule appointments and meet legitimate business and legal record-keeping needs, then delete or de-identify them.

10. Children's privacy

This website is not directed to children under 13, and we do not knowingly collect their information online. If you are a parent or guardian and believe a child has submitted information through this website, please contact us to request its deletion.

11. Links to other websites

Our website links to other websites and services, including the patient portal, Privia, social media, Substack, YouTube and partner apps. We also display third-party content. Those services have their own privacy policies, which apply when you visit or use them.

12. Your choices and rights

You may contact us to ask to access, correct or delete information you submitted through this website. We may need to verify your identity, and legal or record-keeping obligations may limit deletion. You can opt out of text messages and manage cookies and browser storage as described above.

13. Your medical records and HIPAA

Our handling of protected health information in your medical record is described in our Notice of Privacy Practices. This website policy covers information collected through the website and does not replace that notice.

14. Changes to this policy

We will post updates to this policy here with a new effective date.
